Endovascular aortic aneurysm repair (EVAR & TEVAR)

EVAR (Endovascular Aneurysm Repair) and TEVAR (Thoracic Endovascular Aortic Repair) are among the most advanced methods for treating aortic aneurysm without open surgery and are now considered the preferred treatment standard in many high-risk patients.
These specialized procedures are performed by Dr. Seyed Mohsen Jahromi Moghadam
(Interventional and Endovascular Vascular Fellow)
in advanced specialty hospitals in Mashhad using modern angiography equipment.
What is EVAR? Endovascular repair of abdominal aortic aneurysm
EVAR is a minimally invasive method for treating abdominal aortic aneurysm (AAA) without opening the abdomen.
In this procedure, a stent graft, a metal tube covered with special fabric, is introduced through the groin arteries and placed at the aneurysm site to:
- Redirect blood flow away from the weakened segment
- Remove pressure from the aortic wall
- Significantly reduce the risk of aneurysm rupture

Steps of EVAR
- Patient preparation with general anesthesia or regional anesthesia, depending on the patient
- Vascular access through the groin with very small incisions over the femoral arteries
- Insertion of catheter and stent graft
- Precise guidance with fluoroscopy, or live X-ray
- Deployment of the stent graft at the aneurysm site
- Removal of instruments and closure of the entry site
Indications for EVAR
- Abdominal aortic aneurysm diameter greater than 5.5 cm
- Rapid aneurysm growth (more than 5 mm in 6 months or more than 1 cm per year)
- Presence of symptoms, such as abdominal or back pain
- High-risk or older patients who are not good candidates for open surgery
Possible complications of EVAR
- Bleeding or hematoma at the access site
- Infection
- Endoleak, or blood leakage around the stent graft
- Stent migration or obstruction
- Vascular injury
- Contrast-related kidney failure
- Cardiovascular or cerebrovascular events, rarely
Hospital stay and care after EVAR
- Hospital stay: 1 to 3 days
- Avoiding heavy activity
- Regular medication use
- Periodic CT scans to evaluate stent function
- Urgent medical attention for abdominal pain, fever, chills, or sudden pain

What is TEVAR? Endovascular repair of thoracic aortic aneurysm
TEVAR is an advanced method for treating thoracic aortic aneurysm without open chest surgery.
In this procedure, the stent graft is guided to the thoracic aorta through:
- The femoral arteries in the groin
- Or the subclavian artery under the collarbone
It is then deployed at the aneurysm site.
Steps of TEVAR
- General anesthesia
- Vascular access from the groin or subclavian region
- Precise guidance of the stent graft with advanced imaging
- Fixation of the stent inside the thoracic aorta
- Completion of the procedure without opening the chest
Indications for TEVAR
- Thoracic aortic aneurysm larger than 6 cm
- Rapid aneurysm growth
- Patients at high risk for open surgery
- Traumatic injury of the thoracic aorta
- Selected aortic dissection cases
Possible complications of TEVAR
In addition to complications shared with EVAR:
- Stroke
- Spinal cord injury, paraplegia, rare but important
- Voice change due to laryngeal nerve injury
- Breathing problems
- Low blood pressure
Hospital stay and care after TEVAR
- Hospital stay: 3 to 5 days
- Careful neurologic and respiratory monitoring
- Regular imaging follow-up
- Urgent evaluation if the following occur:
- Limb weakness
- Shortness of breath
- Dizziness
- Voice change
